It is most definitely a code error. A cardinal rule of UNIX
programming is that if malloc/vm_alloc/free/etc. fail, you have done
something wrong.
A C++ mailing list would be a much better place to ask these sorts of
questions, since you are not only using C++ but you are doing
something very non-standard.
