Are there any rules of thumb as to when a preference change happens immediately and when it applies later (such as next open document)? Somewhat of a reverse way of asking this: Is there any standard for when to apply a preference to the active document vs to the next opened document?

Users expect preferences to take effect as soon as is logically possible. This is actually quite easy if you're implementing preferences using bindings.

A preference is just that -- a personal preference. If you tell somebody at a deli that you don't like mayonnaise on your sandwiches, would you like the person behind the counter to take that into consideration immediately or ignore it until the next time you exit and enter the store? :)
