default nib on document based apps

How does cocoa determine what the default nib to load
on document based apps?

By messing with PB on a document based project I
noticed that there are extensions you can associate
with a given class.

Furthermore how do you know that your nib_loaded
message will be invoked?  I was looking for the
documentation
on this and if anyone has a link handy that would be
great.
