Customize NSAlert?

Is it possible to easily customize an NSAlert?  I have an alert that I need to add a checkbox to ("Apply to all").

I can see no way of customizing the panel which NSAlert uses, nor can I see an obvious way of getting NSAlert to use a panel or any additional controls that are defined in a NIB.

Is my ownly option to create my own window from scratch?  I guess that means creating a panel that looks like an alert panel, with an icon that looks like a regular alert icon, creating a new class or a bunch of methods to catch the button actions, then wire the whole thing up in IB.  That seems like a lot of work just to get one button.
