To address this quickly:

Yes: Tell us what works for you.
It’s good, but: Report typos, inaccuracies, and so forth.
It wasn’t helpful: Tell us what would have helped.

Is apple using this really?

Yes, really! Many fixes in the recent release have come directly from this feedback, and more will in the future.

I am curious, how many people files a bug about documentation this way or any other ways?

I don't think I can give actual numbers, but a fairly large number. But not as much as one might expect given traffic here... This is why I (and others) continue to urge people to send in feedback. That's the only way to guarantee it'll be addressed in some fashion. I and others submit some bug reports from issues that arise on the list, but in general they're likely to be given lower priority than something raised directly.

To take this opportunity to go off on a tangent (but related to comments made elsewhere in this thread): Issues will typically be given higher priority if they're well formulated and provide actionable requests (well, except for the occasional, "w4nt mor pr0n" that comes through...). Spelling mistakes and other typographical errors are straightforward, but "This doc sux" doesn't give much indication of where there's scope for improvement. ("This doc rox" is pleasant enough to receive, but it's also helpful to know more specifically what was good about it too.)

It would also be nice if apple added the page name somewhere on this 'Document feedback' page instead of the overall document name. I know its hidden but it would be nice to show the real place where we are reading.

I'm not sure what you mean here? The page information is sent as part of the feedback -- is there something else you'd like to see in the feedback form itself?
