Subclass NSScroller to perform your own scroller tray, button, and knob drawing. Wrap your NSTextView in an NSScrollView and set the scroll view's scroller to your custom subclass.

Honestly, I'm /really/ disappointed that Apple gave us the HUD panel but not any of their HUD interface elements to go with it. It just makes no sense. Please file an enhancement request!
