I don't know about an official answer, but empirically, I've found that
the real NSScroller determines whether it is horizontal or vertical by
the frame that's passed in when it is first initialized. Changing the
frame after its first initialization actually has no effect on its
orientation. (Perhaps this helps in cases when windows are resized to
super-small sizes.)

- (void)drawKnob;
- (void)drawKnobSlotInRect:(NSRect)slotRect highlight:(BOOL)flag;
- (void)highlight:(BOOL)flag;
is the way to go.
