I found ImageAndTextCell in Apple's sample code a bit useless, though I forget now what the problems were I had with it. It's possible I got hold of an old version too. But anyhoo, I wrote a simple replacement that I used for showing a folder next to the name in an outline view, and it's part of this little project here:

If it works for you, feel free to use it.
