If you are using WO 5.3 there is a known issue. The installer does not install the template files used to generate class files from EOModeler. The easiest fix is to copy the files from a WO 5.2.4 installation.

The files should be installed in:

/System/Library/PrivateFrameworks/EOModelWizard.framework/Resources
