Below are a test .xinitrc file and the relevant console output. Any suggestions would be appreciated.

I am using Apple's X11 0.2.1 on Mac OS X 10.2.4.

Apple has added a new way of mapping the keyboard based on OS X system defaults. Unless this is disabled, I believe that it overrides some xmodmap mapping. Could it be doing the overiding after the xmodmap in the .xinitrc? The FAQ or archives from here should show how to disable the new mapping.
