I use ctwm (everwhere, since 1987, and have no intention of changing ;)
which satisfies all four, with one caveat: it displays origin/extent
during move/resize in a teeny window at +0+0 which ends up obscured by the
menu bar. I changed one line in the source to move it down 22 pixels and
recompiled, and have been delighted with it on OS X ever since. (I also
modified it to reject first click when RaiseOnClick is set, which behaves
better when clicking in the middle of Emacs to raise the window [so the
cursor doesn't arbitrarily follow the raising click], but that's
tangential.)
I've put screenshots of move, resize and changing virtual desktop
occupancy on a window here (the small window top-left is the console and
the buttons next to it are the desktop switches):

along with a tarball of my trivially-modified sources. (I can send you my
.ctwmrc as a starting point on OS X too, along with the mini icons I use
for the title bar buttons if you like the way mine looks.)
