I don't think this issue has anything to do with writing man pages.
Rather, I believe that those strings surrounded by __ on either side
are supposed to be filled in by some external configuration process
before the man pages are installed into the distribution.
I don't have the appropriate sources here, but in the xserver sources
you can see and example of this in doc/Makefile: See the variable
MANDEFS and how it is being used.
So there's your problem: You need to go back to the source where this
man page came from and make sure it gets processed right, and that the
processed man page makes it into the distribution, not the unprocessed
one.
