I haven't isolated the repro yet, but if you're looking at a source file and use the "switch to source/header" command, then sometimes Xcode will do so in the *current* window (rather than opening a new window), meanwhile keeping the previously file open (now making it now inaccessible/hidden). If you this happens enough times, when you go to close a file window, you'll get prompted to save all the dirty files that have been made inaccessible. It's as if the current OS window is being recycled (rather than a new one being created) and the previous file association is still active (but hidden). I saw this bug in 2.5 as well but was hoping it was gone in 3.1, hrm. Has anyone here isolated the repro or already filed a radar on this? To be clear, I'm using 3.1 using the "Condensed" style layout with codewarrior key bindings.

Looks like "Open counterparts in same editor" preference in Xcode -> Preferences -> General tab is checked for you. This preference is checked by default. You might want to uncheck it if you want to open counterparts in different windows.
