Debugger skips/ignores breakpoint?

I realize breakpoints (and debugging in general) can be a little
problematic when optimizations are enabled, but with a -O0 project,
what causes the debugger to skip all the breakpoints? It's only doing
this on one project. No, the breakpoints aren't disabled. When I
debug the project, all the breakpoints (big fat blue looking arrows)
getting overlaid with a smaller inset yellow arrow automatically.
What does this mean? If I click the breakpoint it goes back to faded
blue and then toggles back and forth between faded and dark blue
(disabled and enabled). The yellow arrow only returns when I run again.
