What's the difference between these two roles?

I think AXMatrix is obsolete. You should never see one. You'll see AXLists instead.

Additionally, what if you have a custom control that doesn't fit any of the predefined roles, can you custom define an AXRole or do you just give it your best shot?

You should send me an email describing your issue. I will forward it to an internal mailing list which use to discuss such issues (not sure if I can give out the addr).

In general, we don't want people creating new roles. The problem is that assistive apps won't be expecting new roles and won't be able to handle them all that well. Sure, they're self describing to some extent - but usually not enough to really be handled well.

There are, of course, exceptional cases, but realize that creating a custom role is tantamount to asking all the assistive apps out there to have special case code for your app.
