I can't help you with this. My experience lies entirely on the assistive
application side of the equation. I know nothing about accessorizing custom
elements.
I note, however, that the "Accessibility Notifications" section of the
Accessibility Programming Guidelines for Cocoa document is very short. It
does not suggest that there is anything special you need to do to make a
custom element issue notifications. In those circumstances, my normal
assumption is that I've made a very simple programming error somewhere in my
own code. I always treat the possibility that it's an Apple bug or
documentation error as the least likely explanation.
