Use the Accessibility APIs available in Cocoa, then your application
will be available to VoiceOver users.

To get started using VoiceOver in Leopard, start VoiceOver (in System
Preferences > Universal Access) and then type Ctrl+Opt+Cmd+F8. After
you've gone through the quick start, try Ctrl+Opt+F7 for more.

I'm working on a program for the blind and visually impaired, and I'd
like to get completer control of the keyboard. Is that possible?
I'm handling keyboard events, but some of them are grabbed upstream

some of the things done are less than optimal for a user who can't see
what's going on. It'd be really cool if I could keep the sound from
being muted or the app from quitting. I'm not afraid of Quartz, but
I'd
just as soon not patch the kernel...
