Fraser,
There isn't a special setting to use VoiceOver solely for that
purpose. The closest would be to turn on VoiceOver, and turn off as
many of the announcement options as possible in the VoiceOver Utility
application to keep what VoiceOver speaks to a minimum.
I would encourage you to file these enhancement requests in our bug
reporting system. You must be a member of our free ADC developer
program to file bugs, using Bug Reporter. Details are at this URL:

Note when filing bugs, to submit one bug per request.
Thanks,
James

Is there any way to use VoiceOver solely to provide feedback while
typing? If Apple included a "Speak Keys" tick box for the
"difficulties with initial or repeated keystrokes" item in Universal
Access, I'd turn it on.
The option to turn on Slow Keys selectively would help me, too.
Preventing accidental "returns" specifically would increase my
productivity.
