I was running into this issue, too.
At one time it was working almost great (by even registering for the tab title changes) but when I navigated in Safari by bookmarks, dragging tabs around and drag tabs from one window into another, I gave up.
Instead, I implemented a check based on a timer and compare the title strings.
It would be great if this will be resolved in the near future.
