Application added to accessibility not staying clicked

I have a user reporting an issue with my application (on OSX 10.11.3) which uses the Accessibility API. When they go to perform an operation, it takes them to System Prefs ->Privacy and Security if they have not added the application to the list of approved Accessible App. They check the box for my application in Accessibility. They then repeat the function,which takes them back to System Prefs and the Application’s box is unchecked. They can’t seem to get the checkbox to stay enabled so they can use the application. They're stuck in a loop.
I can’t reproduce the issue on over three other OSX 10.11 instances. I can recreate the issue if I change my application’s bundle identifier in the info.plist. I did this because I thought the Accessibility database was corrupt. But Otherwise there would be no reason to do this.
