Yes, Accessibility Inspector displays what I assume is Apple's intended
hierarchy of UI Elements, and it does show the hierarchy that I expect. So the
bug lies in the functions I described that ignore any menu bar containing menu
extras. Other functions in the Accessibility API, which I also described in my
original post, show the same hierarchy that Accessibility Inspector shows. So
until Apple fixes this problem, developers who care about menu extras should
pick and choose their functions carefully with this in mind.
