Have you tried saving the page as an EPS and distilling that? A saved EPS
from Quark crops to the page dimensions (Unless you add bleed in the 4.x
save as EPS dialog). The disadvantage of this is lost control over your
postscript. But, for automating PDF's for proofing only, it works fine for
me.
