Form field data into database

Greetings all,

I am looking for suggestions on the best way to transfer data from a
form to a database using AS. More specifically, I would like to create a
database (possibly in MS Excel?) that would allow such functions as
finding duplicate items and calculating the sums and averages of
columns. The data would come from possibly a PDF or HTML form with
fields that are filled in and selected radio buttons. The entire process
can be performed on the client side or the data from the form could be
transferred via email (this is strictly for departmental use, so no
webserver is involved). I am new to all of this and feel like I am
probably biting off more than I can chew, but I would appreciate any
guidance that can be given (simple terms for a simple mind, please).
