PDF Filter XTension (from Quark and for 4.0x) can do it (expor PDF), but you
need always a third-party product (Acrobat Distiller with PDF Filter, and a
postscript desktop printer).
PDF Filter prints a postscript and then distilles it.

The easy way for me is create the links from Adobe Acrobat 4. If I have
"
http://www.pdf.com/" or "
mailto:email@hidden", Acrobat makes the hyperlink
automatically.
Tools > Web links... > Create...
It's easy for me because I can control all the process of my pdf: I choose
my printer. If I need generate a postscript I'll choose a ps converter. If
not, I'll use directly PDFWriter, PrintToPDF... (PrintToPDF, for example,
generates Bookmarks).
I'm finishing because I'm seeming look like Quark/Adobe's comercial agent.
