1st Using Prefab Player I had it automatically enter a page number into the
dialog box that appears in Quark when placing a pdf.

2nd Using Acrobat 4.05 full version I would slice the document apart into
single page pdf's with the same name as the original and a .1.pdf extension
at the end to keep track of what's what.
