By far, the best is Dialog Director
<http://www.hylight.demon.co.uk/DialogDirector/>. Dialog Director lets you
define dialogs using a resource editor such as ResEdit or Resorcerer, and
therefore allows you to create any kind of dialog item a Mac OS software
developer can create.

If you aren't into editing resources, try Dialog Studio
<http://macscripter.net/dialog_studio.html>, which lets you design your
dialogs in a WYSIWYG editor.
