Do you mean editable by hand in the document? Then you need to create
something like a photoshop or fireworks document with layers (for
simplicity) or at least with editable elements.
Photoshop X is AppleScriptable and In Fireworks Mx you do this using ECMA
script (nee JavaScript).
One thing just in terms of producing non-editable images, would be to use
Smile with its new pdf generation library. You could composite your labels
over the image in pdf.

like Script Editor (in that you can have script and result windows), but has
neat Script Debugger-Type features such as a handler popup. They just need
to include a drag and drop UI editor and we have a workable Hypercard
substitute.)
