Multiple Buttons

I'm trying to create a script that asks the user which Client they want to
run the particular script on in Quark. Currently I have them using a "Fill
in the blank" style. I would love to be able to list all the clients(around
15) with check boxes or each client as a button. How would I do this? I've
tried and only gotten errors.

Any help would be appreciated.
