Adobe's PDF format has a feature that allows one to add stuff to the end with absolutely no modifications to the original. It is intended for such things as mark-up and encrypted signatures.
In any case it is fully documented in paperback and in PDF freely downloadable form the Adobe site. Yeah - there's a learning curve but I think it might be worth it.
You would need to provide blank spaces in a master PDF document and fill them in in a specially tailored addendum which would be simply concatenated to the master PDF. The operation could be part of a real time transmission like a CGI process.
The original can be deflated or otherwise compressed while the addendum can be all TEXT.
