This isn't an Applescript solution nor is it a PageMaker solution, but in the
tradition of "use the right tool for the job," might I point you to

It may look a bit intimidating at first, but it is fully capable of doing
what you need (and much more), and pretty easy and intuitive once you have
played with it for a few minutes. I wrote it using Javascript almost 4 years
ago, and it has proven very solid (at least with Netscape Communicator 4.x; I
haven't used it with IE). The "custom" feature isn't implemented yet, but
UPPER/lowercase is, including a mechanism for first character uppercase and
others lowercase (takes several easy steps).
Let me know if this works for you (off list, since it's not an Applescript
answer).
