The System Events application must be running for Folder Actions to work.
You'll find it in /System/Library/CoreServices. Drag it onto the System
Preferences Login window so that it will launch every time you log in.

In the course of debugging folder action scripts, I've encountered cases where the FA script caused System Events to crash silently. This effectively caused all subsequent folder actions to stop working for no apparent reason. Now, when debugging a folder action, I use a stay open script to periodically check to see if System Events is running and, if not, launch it.
