In the original post, Joe cited some app that (if I remember correctly) left an icon in the right hand side of the Dock, presumably even it it wasn't running. Unless that app added itself to the login apps and is actually running minimized, the only way I can think of to do that is to place a document (with a custom icon?) in the right side of the Dock. If the file type and maybe creator codes or at least the extension is right, this would start up the app when clicked.

I don't think so, but you can make an app stay in the Dock either by
dragging its icon there (to the left side) while the app is not running, or
if it's already running, choose "Keep in Dock" from its Dock menu.
