To rebuild the desktop in 10.2 and up, you have to delete the following
files.

~/Library/Caches/com.apple.LaunchServices.UserCache.csstore
/Library/Caches/com.apple.LaunchServices.LocalCache.csstore

I'm not sure if you should reboot or just logging out and logging back
in.
