Adding a doc icon flag

I have done a little looking for this, but so far I have not had any luck
(I think that may be becuase all the key words are too general).
I am wondering what is involved in adding a flag (like the little red ball
on Mail.app that tells you how many messages you have) to an icon in the
Doc.
The terms "Flag" "Doc" and "Icon" do not result in a very focused search.
Is there some other term I should use, or better yet a pointer to a
resouce that goes into this?
