A dark black rectangle appears around the currently selected UI element.

You probably accidentally typed command-F5 and have inadvertently turned on the VoiceOver assistive technology for the visually impaired. ( http://www.apple.com/macosx/features/voiceover/ ). Type it again to turn it off (or go to the Universal Access pane in System Preferences.)
