I don't know why it doesn't work for you; I copied and pasted the script into text editor and ran it, and it worked as it should.  All the entries have a label of "work."

BTW, it also shows a photograph in the address book; I have no idea how it got there.

Just tested it again, and it works okay for me too.  Mac OS X 10.4.2, AB 4.0.2.

If you have a .Mac email address, you can log into the .Mac website, and upload a picture in your email account settings.  After doing so, the picture will appear automatically in AddressBook when someone enters your address.  ;-)
