In OS 10.4.5, at least, 'keystroke' works even without "Enable access for
assistive devices" enabled in Universal Access. Other commands, such as
'click' errors, but 'keystroke return', 'keystroke "a"' all work.

I have got 'keystroke return' to work (accidentally, first time) in
Microsoft Word's Progress Gallery (the OK button that dismisses it) and all
sorts of things in Apple's Address Book: with "All" previously selected,
'keystroke "s"' will select the first group whose name begins with "S", or
with the Edit button in operation 'keystroke "s"' enters an "s" in the
highlighted text field.

Useful, but could be dangerous, especially for dialogs requiring a default
button to be clicked for permission. (It doesn't work for 'display dialog'
though: that's still waiting for itself to be completed the normal way
before it will move on to the next command that would do the keystroke.)
