Paper and Registration can't be removed from any document, so you're always
going to get them included -- you just have to work around that. I don't
know where your White came from -- probably something you imported.

As for the empty names, they represent colors applied using the Color
Palette -- each time a different color is used this way, a new color is
created. It's unnamed so it doesn't show in the Swatches Palette.
