The problem came back!

So I ran...

 Onyx 1.7.9 -> Maintenance -> Launch Services database -> Rebuild -> Execute

...and now it's gone again.  This time, I think I understand a bit better why.

I had not even heard of a Launch Services database, and now I have a rebuilt one!

It's a minor drag having to OK every app's first launch again, but, hey.

Thank you all for your ideas and encouragement.
