Being able to say { keystroke "O" } is just a convenience feature.
The command really means "simulate pressing the O key" - which should
trigger whatever that key does when you press it.  "o" normally, "O"
with shift or caps lock, control-O with control, etc.  You can say

keystroke "o" with shift down

for instance.
