If I set ikey to "Generate event when keys are pressed" the problem occurs every time regardless of key sequence.
If I set ikey to "Generate event when keys are released" the problem occurs like your example.
NOTE: this is the only iKey command this occurs with. If I launch my "Create-Make RAM Disk" AppleScript app by double clicking, it will display that dialog if I hold down the command key while double clicking.
