It only finds 18.

But at least they're exactly the same 18 that tag finds ;-)

A Spotlight search for tag:red in ~/ yields 609 items.

Spotlight searches are odd things. To keep them fast, queries return an initial batch, and can then keep adding extras in updates. This works fine in a UI, but not so good here, as you can see. I'll see if I can add that in, but at the moment I'm dealing with a system migration...

You might do a comparison with mdfind.
