PDF TOC

We can programatically generate PDF Table Of Contents with AppleScript.

But…I can not generate TOC from zero
Now, there is need to make dummy TOC by Adobe Acrobat.
Is there a way to create TOC?

Now:
  step1: Generate PDF from Application (Ex. Keynote.app)
  step2: Make TOC by Adobe Acrobat (dummy TOC is available)
  step3: Remove Toc by AppleScript
  step4: Generate Too by AppleScript
