I’ve set up two transparent, borderless buttons over a dumb (as you put it), stepper. Works really well.

It shouldn't. Change at 11, not 10, or re-think your plan. Steppers just aren't designed to have different increments depending on where you press them.

When something like this is difficult, it's a hint that you're fighting the rules of the UI. That's not to say you never should, but it should give pause for thought.

If you insist, you'll have to keep track of everything yourself, just using the stepper as a dumb button. Indeed, you might as well just have two buttons.
