Fellow UG members, you're all working too hard!
Here at CIDER, we have a sign-in sheet that we pass around for attendance (really just a head-count) during the meeting, but we number each line. It's on a clipboard and has an attached pen. At the end of the meeting, we find a random number generator that has definable upper and lower bounds, and keep drawing numbers until we run out of prizes. The display from the RNG is shown on the projector.
You must be a member and present to win. If the number drawn matches the line you signed in on, you win! If we have multiple prizes, the winner gets their choice from what's available.
This way, we don't have to print/distribute/collect/draw from/dispose of slips at every meeting.
I'll admit that it doesn't have much for drama or ceremony, but it is the easiest way we could think of.

There is a 'pre-ticked' box stating that winners agree to have their Initial, Surname and Suburb published on our website (and/or in our magazine) and if relevant their email address may be passed onto our sponsors (in order for them to supply some prizes direct to the winner). There is often a box to tick to OPT-OUT of 'marketing' emails from us.
