I have seen this occasionally with iOS 6.1.2 on iPod Touch 5th and iPhone 4S. Every time I've seen it, turning Bluetooth off and on again from Settings was the only thing I could find to fix it. You mention the simulator -- are you running only on the simulator, on a real device?
I haven't raised a bug with Apple yet as I haven't been able to reproduce it sufficiently reliably to rule out a problem on our end.
