Write your own speller service -- it is documented quite well.
There are just two problems:
(i) probably due to the blasted Carbon entrails, the spellchecker does not
properly understand non-Roman characters: your speller service can check them
all right, but you can't add them to the User's Dictionary;
(ii) Apple does not bother to run the service daemon -- you have to DIY.
Otherwise it works well. and the new API is much better than the ancient
NeXTStep one.
