Unfortunately, it's been like this for several days and I have
restarted the machine several times, and it's still not coming
up. The application icon is coming up just fine everywhere it
should...
These are the Desktop Database equivalents, AFAIK, in
$HOME/Library/Preferences:
.LSApplications_Backup
.LSClaimedTypes_Backup
.LSSchemes_Backup
LSApplications
LSClaimedTypes
LSSchemes
You can simply remove them.
