I'm trying to dynamically change the title of one of the top-level
menus, i.e. a menu in the Menu Bar, using [NSMenuItem setTitle:].
Though the title seems to change internally, it doesn't show up in
the UI. This works fine in changing a menu item within a pull-down
menu, but there appears to be something special going on for the
menu bar. Does anybody have any insight on how to get the changed
title to display properly?

This does seem to be a problem and it won't be fixed for 10.1. I've entered a bug for it.
Unfortunately, I can't suggest a workaround.
