I'm quite aware of these solutions. :->
I want a readme file that is separate from my app (fairly common practice). It discusses basic issues like how to install the software, where to find resources like doc and downloads on the web, etc., that users may want to read about before they ever run the app.

Please include it in your app, IN ADDITION to having the separate file. I hate having to save separate readme files.

Hmm... maybe i'll file a feature request for a "view readme" context menu in the finder... That would solve the "users don't want to run executables without reading readme" problem, and the "apps should be in once icon" problem.
